 Resolution : 

To set up the form go to SLAEVNT

In the event field enter ADD

Description  = Campus Visit Day – 04-17-2009

System = S

Event Type = RECR

Contact ID = 929103596

Address Type: ST

Phone = 6783595354(This will automatically populate)

Campus = A

College = GC

You then hit the save button. It will automatically assign the next number.

Change to the Functions tab and assign the date and time. Hit save again.
